Concussion Information, Resources, and Web Links
Required form for parents to report their child’s head injury to the school, if it occurs during the summer or school year away from school, during out-of-school sports, or during school sports. This report should be completed by the parent and given to the school nurse. Also, the child should be evaluated by the pediatrician as soon as possible after the injury.
